At a Kaiser Permanente premier SNF, we know you before you even arrive.
Your Kaiser Permanente team will decide if you need care in a skilled nursing facility. On average, patients stay a maximum of one to three weeks.
When you are about to be transferred from the hospital to a premier SNF, your primary care physician (PCP) is notified and your SNF health care team gets ready for your arrival. Information about your condition can be easily shared between your PCP and the SNF health care team, allowing them to coordinate with any specialists, case managers, and therapists assigned to help with your recovery.
Our goal — just like yours — is to get you better and back in the comfort of your home as soon as possible. Because the care is so well coordinated, we have found that members who stay in our premier SNFs are often able to go home sooner than those who stay in other facilities.
At a premier SNF, your health care team will be made up of your Kaiser Permanente physician, Kaiser Permanente case manager, SNF rehab team, your nurse and the facility social worker. They will plan and coordinate your care from hospital to SNF to home. Having quick and direct access to your electronic medical records means they know everything about your plan for care. It’s the same information that’s available to your PCP and the hospital where you’re being treated.
Being connected like this means that everyone involved in your care stays up-to-date on your condition, medicines, tests, and therapy at all stages of your rehabilitation. Your health care team will work with you to coordinate any follow-up care and visits.

At a premier SNF, you’re more than just a patient. You’re our member, and your Kaiser Permanente health care team wants to do everything possible to maximize your recovery.
Your Kaiser Permanente health care team knows who you are and why you’re there, before you even arrive. They’ll answer any questions you may have.
You’re seen at least weekly by your Kaiser Permanente physician. Your Kaiser Permanente case manager sees you even more often than that. Each one oversees a specialized part of your care.
Your Kaiser Permanente physician and case manager are closely involved in your care plan.
Your Kaiser Permanente case manager is there for you to talk with, and to help you with your transition from the SNF once you’re ready to be discharged.

Not all skilled nursing facilities can be Kaiser Permanente premier facilities. To qualify, each facility has been carefully evaluated—and is regularly reassessed—for the quality of care and services it provides.
